Faculty Practice:NP practice in mobile vans and public housing developments

Abstract:
The Jordan and Harris Community Health Center is a HRSA funded project that brings primary nursing care to three public housing developments in the city.  Opened in 2011 by the University School of Nursing, the project currently serves 25% of the total population residing in these developments.  The population served is one of the poorest in the state and is medically underserved.  A community advisory board that is comprised of residents oversees all program activities.  A cadre of Community Health Workers was also hired from within the housing developments and they report directly to the RN Coordinator who manages the project. The CHWs provide outreach on a daily basis and are the direct link to the residents of the community. The program is demonstrating improved health outcomes for diabetes, and hypertension. Preliminary data suggest that frequency of visits to the Center’s RN correlates to improved chronic disease management. The program has had over 10,000 encounters in two years.

The Children’s Health Project or School of Nursing Mobile van was established in 2007 with HRSA funding.  This mobile van brings primary APN services to the communities served by Jordan & Harris.  The van currently treats approximately 1500 patients annually.  In addition to the J&H sites, the CHP serves Covenant House, a shelter for homeless teens and La Casa De Don Pedro a community organization that serves the Latino Population.
